Reconstruction of Posterior Cruciate Ligament of the Knee Using Free Patellar Tendon Graft(Clancy)


Abstract
It has universally been accepted that the posterior cruciate ligament is the prime stabilizer and the most important ligament of the knee and so its rupture should be repaired immediately if good stability and function of the knee is to be restored.
In this report reconstruction has been tried to correct the knee instability with rupture of the posterior cruciate ligament, using a free graft of one third of the patellar tendon with its tibial and patellar bone attachments. This procedure was done in five cases of acute tear of midsubstance of the ligament and two cases of chronic rupture.
At the follow up of postoperative one year, the results were good to excellent in all five cases of acute reconstruction, while the instability remained, though lessened, in two cases of chronic rupture.
This study has shown that the free graft of medial one-third of the patellar tendon is indicated for reconstruction of the substance tear of the posterior cruciate ligament and can achieve excellent static and functional stability of the knee.
